• Was another work on breast surgery really necessary? Well, that’s a good question. It’s true that many titles are already available on the market. If we consider just the books that have been published recently, I think that, two or three books on this subject have been published last year. Along with my friend Querci Della Rovere, I have been involved too as co-author in a work dealing with oncoplastic breast surgery and I have recently co-authored some other texts with my friends Scott, Speare, and others as well.
• These titles provide of course a detailed description of the topic we are dealing with – namely breast reconstruction surgery – and yet, thinking about the work I have done in these last years with our school of reconstructive surgery, and thinking about how many surgeons, from all over the world, have come to see us at work and to get a first hand experience of our surgery techniques, well, thinking about all that, I have realized that by reading a book or by looking at the illustrations of even the most beautiful atlas of anatomy and surgery one can certainly learn much, but one cannot get the actual know-how that a real video with a three-dimensionally vision can provide.
• This is why my team of surgeons and I and have decided to create something new, I mean something useful and practical for all surgeons dealing with reconstructive surgery for the first time, that is: graduates specialising in reconstructive surgery, but also general surgeons, who sometimes have to face mastectomy and immediate breast reconstruction with tissue expanders, and even specialists, who want to improve and refine their techniques to fully master their expertise in breast reconstructive surgery.
• What can you learn from these DVD? You will have the chance to use a multimedia video, or rather a series of multimedia DVDs that will enable both beginners and experienced surgeons to actually see all the steps you should perform, from the patient’s physical exam,
• to the operation planning and the drawings, up to the surgical decision that, as I keep on saying at congresses, must be taken before the mastectomy itself. In fact, a good reconstruction starts from a good mastectomy and achieves its goal by planning first the initial reconstructive step, and then the second step, which could be the replacement of the prosthesis with a permanent implant, any adjustment and refining, or the reconstruction of the nipple.
• As you may imagine, to understand all that through a plain text is rather difficult. On the contrary, it can be easily understood through a series of multimedia videos where one can actually see, for example, how the drawings should be made on the patient, why the surgeon should measure and decide the kind of intervention to be carried out also in the contralateral breast, in order to achieve a good result, how the patient should positioned on the operating table, what kind of tools and techniques should be used, and above all the tricks required to achieve a good final result. In my opinion, this is the right tool we must offer to other surgeons.
• I think it’s a duty for us to put at their disposal our expertise with such a subject. I strongly believe this is the last piece of the puzzle required, so that breast reconstructive surgery may reach that state of the art that will allow anyone to reproduce and get the best results. Our ultimate goal in fact is to improve the quality of life of our patients.

• Perché un nuovo testo sulla chirurgia ricostruttiva della mammella ? In effetti, la domanda è pertinente conoscendo i testi usciti recentemente, mi pare che nell’ultimo anno ne siano usciti due o tre, e i testi che già sono comparsi in letteratura o nuove riedizioni degli stessi.
• In effetti, io stesso ho partecipato come autore insieme all’amico Querci Della Rovere ad un testo di oncoplastica chirurgica proprio della mammella, e sono stato autore in altri testi con gli amici Scott Spear ed altri ma, in effetti, rivalutando questi libri, che danno ovviamente un’ampia descrizione di quello che è il tema che dobbiamo trattare, e cioè la chirurgia ricostruttiva della mammella.
• Ebbene dal lavoro che ho svolto negli ultimi anni con la scuola di chirurgia ricostruttiva e dalla frequenza di medici chirurgi italiani e stranieri che vengono in reparto e che vedono l’attività che svolgiamo e soprattutto toccano con mano la chirurgia, ci si rende conto (debbo ammettere anche che anch’io, quando ero studente ho avuto le stesse sensazioni, che leggere e consultare un libro, anche illustrato, o un atlante di anatomia e chirurgia, aiutano sicuramente, ma non danno quel qualcosa “in più” al chirurgo, quella “tridimensionalità” dell’atto chirurgico stesso.
• E allora ci è nata l’idea, insieme al mio staff di chirurgi con cui lavoro insieme, di creare qualcosa di nuovo.
• Qualcosa di nuovo significa dare un’opportunità ai chirurghi che per la prima volta affrontano il problema della chirurgia ricostruttiva, e quindi agli specializzandi in chirurgia ricostruttiva, ma anche ai chirurghi generale, che a volte debbono affrontare la mastectomia e la ricostruzione immediata con l’espansore, o ancora agli specialisti che vogliono migliorare e affinare le tecniche, fino alla conclusione completa del percorso della chirurgia ricostruttiva della mammella.
• Il “nuovo strumento” è l’avere in mano davvero un qualcosa che possa permettere al chirurgo di affrontare il problema dell’inizio del percorso della ricostruttiva, sin da quando la paziente entra per la prima volta in contatto con il chirurgo stesso per un problema di tumore alla mammella e della sua ricostruzione.
• Cosa vuol dire? Vuol dire avere la possibilità di sfruttare non un video multimediale, ma una serie di dvd multimediali, che consentano a colui che per la prima volta, come si diceva, o al chirurgo esperto, di affrontare ogni volta il singolo problema cominciando dalla visita della paziente, al planning, al disegno, alle decisioni su quanto si debba eseguire. Come dico sempre nei congressi, devono essere decisioni prese sin dall’inizio della mastectomia.
• Una buona ricostruzione inizia con una buona mastectomia e fa tutto il suo percorso programmando contemporaneamente il primo atto ricostruttivo ed il secondo, che può essere la sostituzione della protesi con l’impianto permanente, l’eventuale ritocco e l’eventuale ricostruzione del capezzolo.
• Ma questo non è possibile averlo da un testo normale. E’ invece possibile averlo da una serie di video multimediali nei quali, in effetti, si può capire ad esempio, come disegnare la paziente: prendo le misure e decido il tipo di intervento da eseguirsi anche nella mammella contro-laterale per programmare la giusta scelta; oppure vedere al tavolo operatorio come è posizionata la paziente, quali strumenti uso, quali sono le tecniche, ma soprattutto i “trucchi” per raggiungere il risultato definitivo. Penso che sia questo lo strumento che dobbiamo dare:
• è nostro compito, dico “noi” nel senso di chirurghi che trattano questa materia, perché credo che sia l’ultimo tassello del puzzle per raggiungere uno stato dell’arte sulla chirurgia ricostruttiva della mammella che permetta a tutti di poter riprodurre e raggiungere dei risultati ottimali. Perché l’obiettivo finale è quello di migliorare la qualità della vita delle donne che dobbiamo operare.
